Form 990 Document Request Checklist

Please upload the following documents to your secure portal in MyTaxPrepOffice:

I. Financial Information:

  • Complete Financial Statements:
    • Statement of Financial Position (Balance Sheet) for the fiscal year.
    • Statement of Activities (Income Statement) for the fiscal year.
    • Statement of Cash Flows for the fiscal year.
  • General Ledger Detail for the entire fiscal year.
  • Bank and Credit Card Statements: Along with reconciliation reports for all accounts for the entire fiscal year.
  • Accounts Receivable and Accounts Payable Aging Reports as of year-end.
  • Investment Statements for all investment accounts.
  • Fixed Asset Schedule: Including additions and disposals (date, description, cost).
  • Loan and Debt Agreements: Copies of new or significant agreements.

II. Operational and Governance Information:

  • Prior Year's Form 990 Tax Return (if not prepared by our firm).
  • Organizational Documents: Articles of Incorporation, Bylaws, and amendments.
  • Board of Directors and Committee Meeting Minutes for the fiscal year.
  • List of Officers, Directors, Trustees, and Key Employees: Including names, addresses, titles, and compensation details.
  • Description of Program Service Accomplishments: For the three most significant programs, including expenses, grants, and revenue.
  • Unrelated Business Income (UBI) Details (if applicable).

III. Contribution and Fundraising Information:

  • Donor Information for any single contributions of $5,000 or more.
  • Non-Cash Contributions: Details of any received.
  • Fundraising Events and Activities: Summary of revenue and expenses.
